New Patients

Are you accepting new patients?

Yes. Plymouth County Primary Care is accepting new adult primary care patients ages 18 and older. Patients can schedule online or call the office for help booking an appointment. Same-day appointments may be available depending on the schedule and visit type.

What ages do you see?

Plymouth County Primary Care currently sees adults ages 18 and older. We do not currently provide pediatric primary care.

What should I bring to my first appointment?

Please bring a photo ID, your insurance card, a current medication list, pharmacy information, and any relevant medical records if available.

Appointments & Scheduling

Do you offer same-day appointments?

Same-day appointments may be available depending on the schedule and visit type. Patients can schedule online or call the office to check availability.

Can I schedule online?

Yes. Patients can use the online scheduling link to request an appointment through Athena. You can also call the office if you need help finding an appointment time.

What should I do if I need urgent medical help?

If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. Do not use the website, contact form, or patient portal for urgent or emergency medical concerns.

Insurance & Billing

What insurance do you accept?

Plymouth County Primary Care accepts many major insurance plans. Coverage can vary by plan, so patients should contact their insurance company directly to confirm benefits, copays, deductibles, and network status before their visit.

Should I confirm coverage before my visit?

Yes. Insurance participation, coverage, copays, deductibles, and referral requirements can vary by plan. We recommend confirming coverage directly with your insurance company before your appointment.

Telemedicine

Do you offer telemedicine?

Yes. Telemedicine may be available for certain visit types when an in-person exam is not required. Patients can schedule online or call the office to ask whether a telemedicine visit is appropriate.

Patient Portal

How do I access the patient portal?

Existing patients should use the Athena patient portal to send secure messages, request medication refills, and view visit information. Messages sent through the old Tebra portal are no longer monitored.

Can I still use the old Tebra portal?

No. Messages sent through the old Tebra portal are no longer monitored. Please use the current Athena patient portal for secure communication.

How do I request a medication refill?

Existing patients should request refills through the patient portal or contact the office. Please allow enough time for refill requests to be reviewed.
