New-Patient Questions
Quick answers about starting care at Plymouth County Primary Care.
Are you accepting new patients?
Yes. Plymouth County Primary Care is accepting new adult primary care patients ages 18 and older in Pembroke, MA.
Patients can schedule online or call the office to book a new patient appointment.
Do I need to bring anything to my first appointment?
Please bring a photo ID, your current insurance card, a list of your current medications, your preferred pharmacy information, and any relevant prior medical records if you have them available.
What insurance do you accept?
Plymouth County Primary Care accepts many major insurance plans. Because coverage can vary by plan, contact your insurance company to confirm that the practice is in-network and to verify your copay, deductible, and any referral requirements before your visit. If your plan requires a referral, that requirement comes from your insurer.

I am already a patient. Where should I go?
Existing patients can call 781-755-8715 or sign in to the Athena patient portal for secure messages, refill requests, and visit information.
New patients who are starting care should book a new-patient visit instead. The patient portal page explains the difference between portal information and the Athena login.
What are your office hours?
The Pembroke office is open Monday–Thursday, 9 AM–5 PM. We're closed Fridays, with a few appointments by request. Saturday and Sunday are closed. Call 781-755-8715 or use the Athena patient portal with scheduling questions.
Can I schedule online?
Yes. On the Athena scheduling page, select "Patient type: New," then choose "New Patient" for an office visit or "New Patient – Telehealth" for a virtual visit. Same-day appointments may be available depending on the schedule and visit type.
The times listed online are not the full schedule. If you don't see a time that works, call the office at 781-755-8715 and we'll help.
Do you offer telemedicine?
Yes. Telemedicine may be available for certain eligible visit types when an in-person exam is not required.
Call the office or schedule online to ask whether a telemedicine visit is appropriate for your concern.
